DESCRIPTION:Join Grant Affleck\, Volunteer with End of Life Choices California\, for an overview of advance care planning documents. He will explain key components of the Advance Health Care Directive; Dementia Directive; Voluntarily Stopping Eating and Drinking Directive; and Physician Orders for Life Sustaining Treatment (POLST). \nGrant is participating as a speaker in The Caring Through the Last Chapter Workshop that is co-hosted by the Southern Caregiver Resource Center and UC San Diego Shiley-Marcos Alzheimer’s Disease Research Center.
