SCRC & UCSD – Caring Through the Last Chapter Workshop

Join Grant Affleck, Volunteer with End of Life Choices California, for an overview of advance care planning documents. He will explain key components of the Advance Health Care Directive; Dementia Directive; Voluntarily Stopping Eating and Drinking Directive; and Physician Orders for Life Sustaining Treatment (POLST).

Grant is participating as a speaker in The Caring Through the Last Chapter Workshop that is co-hosted by the Southern Caregiver Resource Center and UC San Diego Shiley-Marcos Alzheimer’s Disease Research Center. This workshop will also feature other expert speakers from local organizations discussing legal, medical, and other end-of-life information.
