Presentation:

Advance Care Planning

This event is Open to the Public.  Registration is requested but not required.

Advance Care Planning is a process that enables individuals to make plans about their future health care; it is applicable to adults at all stages of life. The presentation first demystifies the terms and documents used in Advance Care Planning such as Living Will, Durable Power of Attorney, and Advance and Dementia Directives. The concept of value worksheets is discussed to help people consider what is most important for their lives and for their health care so they are better equipped to talk with family, friends, and medical providers about their wishes.

The overview of End of Life Options briefly describes choices such as discontinuing or declining medical treatments, palliative care, hospice care, voluntarily stopping eating and drinking (VSED) and California’s End of Life Option Act, also called Medical Aid in Dying (MAiD).

Presenter: Carolyn Smith, Volunteer with End of Life Choices California (EOLCCA), will introduce our organization and its services of personal support and education provided through a dedicated team of volunteers.
